If you’ve been off sick for less than 7 days
If you are off work for up to seven days (including weekends and bank holidays), you do not need to give your employer a fit note from a medical professional.
You can self certify. During this time you should complete a SC2 self-certification form which is available from your employer, alternatively please click the link below for an editable version which you can download or print.
If you’ve been off sick for more than 7 days
If you are still unwell and unable to work after 7 days, you will need to request a sick (fit) note from the practice via an appointment with the GP or Advanced Nurse Practitioner.
